  Athletic Handbook

  Two forms need for 7th-12th grade athletes:

1) Physical/Medical Form - follow link below

The MHSAA rule states that for an athletic physical (above) to be honored for the 2010-2011 school year, it must be dated on or after April 15, 2010

2) Authorization for Athletic Participation (7th-12th)

The above is filled out once per year and is used by GCS Athletic Dept.
